Consent to Occupational Therapy Assessment and Advice

This service provides sensory-informed occupational therapy assessment and advice. It does not provide Ayres Sensory Integration® therapy.

I will always obtain informed consent before providing Occupational Therapy assessment or advice.

Consent includes: - An explanation of the purpose and nature of the assessment - What information will be collected - How findings and recommendations will be shared - The limits of confidentiality

Consent may be given verbally or in writing and will be documented in clinical records.

For children or individuals who lack capacity, consent will be obtained from a person with parental responsibility or legal authority, in line with relevant legislation.

Consent can be withdrawn at any time.
Privacy and Data Protection Policy

I am the data controller for all personal data collected as part of this service.

I collect and store personal and sensitive information only for the purpose of providing occupational therapy services.

Information collected may include: - Personal details - Health, educational, or occupational information - Assessment findings and reports Records are stored securely using password-protected and encrypted systems.

Information will not be shared without consent, except where there is a legal, safeguarding, or professional obligation to do so.

Records are retained in line with professional guidance and data protection legislation and are securely destroyed when no longer required.

Individuals have the right to access their records in accordance with data protection legislation.
Appointments, Fees and Cancellation Policy

Standard Notice Period: 48 hours (this may be amended by written agreement).

Report Writing: Written reports are charged as outlined in the service agreement.

Fees are agreed in advance of any assessment or advice being provided.

Missed appointments without notice will be charged at the full rate.

Additional written work is charged separately unless otherwise agreed.

Payment terms are clearly stated on invoices.
Complaints Procedure

If a complaint relates to professional conduct or competence and cannot be resolved directly, individuals may escalate the complaint to the Health and Care Professions Council (HCPC) or the Royal College of Occupational Therapists (RCOT).

I aim to provide a high-quality occupational therapy service.

If you are unhappy with any aspect of the service, concerns should be raised in the first instance directly with me, either verbally or in writing.

Complaints will be acknowledged within five working days and investigated promptly.

If concerns cannot be resolved, individuals may escalate the complaint to the Health and Care Professions Council (HCPC).
Safeguarding Policy

If I have concerns about a person’s safety or wellbeing, I will follow local safeguarding procedures and may share information without consent where there is a risk of harm or where required by law.

Safeguarding concerns will be recorded clearly and escalated appropriately in line with legislation and professional guidance.
